Toxicology
Results are typically available 24–48 business hours from the time the sample is received at the lab.
Fill out a paper requisition form and include it with the patients’ sample in a specimen bag. Samples may be collected by one of our daily couriers, shipped to the lab, or collected at a provider location depending on the arrangement with Rize. If you’re unsure how to get samples to us or haven’t yet set up your default collection methods, please call us at 541-368-3050, ext. 142.
For toxicology, patients cannot view results. Only their provider/counselor can view results. Clients can, however, receive a PDF with their results via encrypted email or through our online portal.

Samples may be cancelled for a number of reasons, including:
- A custody seal was not used
- The custody seal was broken
- Sample did not include the correct unique identifiers – we require full name, DOB, and sample number.
- Unique identifiers on the sample do not match those on the requisition form
- The lid on the specimen cup was not closed correctly, and it leaked in transit.
- The sample cup was crushed, and it leaked in transit. Shipping boxes prevent this.
Anytime we cancel a sample, we will send out a report explaining the reason.

Screening is like a yes or no answer. You either test positive or negative for a substance. It offers a quicker turnaround time, but results are not as accurate. Positive results may require further testing using more complex methods to see the specific quantity of a substance in a person’s system.
Confirmation is testing for substances within a certain range. Confirmation takes longer due to the high complexity of the test, but results are reliable and accurate with no cross reactivity.
We don’t test for the parent substance, in this case ethanol. We instead test for the biomarker. Biomarkers are present for five days while the parent substance itself stays in the system for less than a day. Testing for biomarkers meets all state testing requirements.
No. We can only identify whether that biomarker is present. Genetics, diet, hydration, and lifestyle all impact how alcohol is metabolized, so there is no way to pinpoint the specifics of an individual’s consumption.
These compounds occur naturally in poppy seeds, which can also be found in some breads and bagels. Eating poppy seeds will generally result in lower levels of codeine/morphine, up to 1000 ng/mL, but it may be best to check ingredients and just try to avoid poppy seeds in the days leading up to a test.
We don’t currently provide STI testing in-house, though we can often accommodate special requests through our testing network. COVID for Individuals
We perform RT-PCR testing, no rapid / antigen tests. Tests can be administered as a nasal swab (self-administered), Nasopharyngeal / NP (administered by a registered practitioner), or saliva.
- The out-of-pocket cost for a test is $75.00 for an individual or $25 for 4 or more people testing at the same time.
- For people using insurance, including Medicaid and Medicare, a doctor’s order is required to prove medical necessity. Co-pay/co-insurance may apply. Please visit the COVID Testing for Individuals page for additional testing and pricing information.
- A doctor’s approval via LabDash is required to prove medical necessity.
- We do not approve a test through a doctor’s order that is not in the LabDash system.
- We will need to create the Ordering Provider account for the Doctor. Have them contact us with their information, and we will send them a login. Once connected, they will be able to approve a test for you. If you or your doctor’s office have any questions on how to get this set-up, please reach out to us at our customer support line.
You should not receive a bill from your insurance company for these COVID-19 tests. Please double-check the paperwork to see if it is an Explanation of Benefits (EOB). If it is an EOB, it will most likely say “This is not a bill” somewhere. If it is a bill, please contact your insurance provider.

- If you don’t already have an account, register and sign in at LabDash.net
- Click “Collect Sample” at the top left to begin*
– Select “Collect” under the name of the patient
– Click “Next” as necessary to navigate to the next steps - Select “Enter Sample Barcode” and type the barcode found on the sticker on your test vial (4 letters, 8 numbers)
- Add collection information
– Select symptoms
– If applicable, choose “How will this test be covered?”
– Select exposure - Select “Verify” then “Next” to ensure all information is correct
- Before submitting the collection, the patient signature is required. You must also type in your name as it is shown.
- If all information is correct, select “Submit Collection”
(You can check the “Samples” tab on the homepage to verify that your test was collected correctly)
*Note: “Collect Sample” must be completed for every test. We cannot run unconnected samples.
You’ll need to log in or create a patient account at LabDash.net. Results will be available on the LabDash website in the “View My Test Results” tab below your name and QR code.
Be sure to check the Junk / Spam folders in your inbox. You can check with your IT support if using a company email. If you still haven’t received the email, double-check the spelling of the email address you used to register.
The time it takes for a sample to arrive at the lab depends on how the test was sent to us. If it was picked up by one of our couriers, it should have been received the same day.
Shipping times with UPS may vary and typically take 48–72 hours. We do not receive shipments on weekends or holidays so you may need to factor additional time into return shipping.
If your sample has not been received by the lab within 7 days of collection, you will need to provide another sample. You can call the lab for assistance at 541-368-3050.
Yes. QR Codes are available once the results are ready in the “View My Test Results” tab. If you need a name correction or additional information about your results, please email covid@rizelab.com or call the lab at 541-368-3050.
Yes. A parent / guardian who is over 16 years old must create the account. Once signed in, you will have the option to “Add a Dependent” below your name and QR code.
We can test any age, as long as they are able to administer the test for themselves (or a parent / guardian can assist).
Yes. You can use one email address to create a primary account, then add another person as your dependent as long as they are with you at the time of testing.
Results from RT-PCR tests can typically be used for travel, events, surgery, etc. Please check the requirements from your specific provider, venue, or travel destination.
Yes. With an order form or signed release form, we can send the results to a specific facility. You’ll need to provide us with the fax number for the facility.
Please be sure the turnaround time listed on our drive-up locations is sufficient for your needs.
COVID for Organizations
You can call the lab at 541-368-3050 or reach out via our contact form. If you use the form, simply select “A COVID Testing Program for my Organization” from the dropdown menu.

Rize couriers are typically available during business hours Monday through Friday. Please call the Courier Logistics team at 541-602-2225 to request specific pick-up and drop-off times. We can work with you to find the best time for your organization.
Once a sample has been collected, it needs to be checked in at the lab within 7 days. Samples received after the 7-day period will be cancelled.
Refrigeration is helpful for preserving the integrity of the sample but is not required from the moment that you take it.

This information will likely be sent to the email address used to create your account. You can also navigate to the Orders tab and search for Status > Delayed.
By looking up the sample in Orders > Status > Delayed, you can locate a barcode and the information written on the sample. Input this information in “Collect Sample” in your organization’s account.
Delayed samples need to be cancelled and disposed of after 7 days.
School Testing Program
All school testing programs will be discontinued at the end of the 2022-23 school year. We will continue to run samples that are received at the laboratory on or before June 30th, 2023.
Rize has partnered with the Oregon Health Authority (OHA) to provide weekly at-home testing to teachers and school staff throughout Oregon.
All Oregon School teachers and staff qualify for free weekly testing provided by Rize and OHA.
If you would like to be removed before the end of the program on June 30th, 2023 please email covid@rizelab.com. All participants will be unenrolled on June 30th, 2023.
The cost will be covered by the OHA.
